For your scout's safety. For sibling safety. For leader safety. For your safety. We absolutely must have up to date medical forms on hand. This is not optional. You will not be allowed to participate if we do not have this form.

We need parts A & B filled out, and a photo/scan of the front and back of your insurance card.  The most critical elements of this form are: Emergency contact info, allergies, medications, and insurance info. 

This form is required for ANYONE that will be at an event, not just scouts.
